Finger Lakes Wine Country Comes to the Library

The Chronic Disease Self-Management Program (CDSMP), is a six-session peer led health education program for adults (18+) with any type of ongoing health condition. Each session complements the health care you may already be receiving. The purpose of the CDSMP is to enhance your ability to manage your health and maintain an active and fulfilling lifestyle. You do not need to have a chronic illness to attend; maybe you know or care for someone who does.
